The Legacy at Home, the largest not-for-profit home health care agency in Greater Dallas, is proud to announce it is the preferred home health provider for Village Health Partners, a nationally recognized primary care practice based in Plano. Additionally, The Legacy at Home is relocating to Independence Medical Village, located at 8080 Independence Parkway in Plano. Village Health Partners is an anchor tenant of Independence Medical Village, a one-stop comprehensive healthcare facility with award-winning physicians and services providing convenient, accessible and high-quality patient care.
Independence Medical Village provides convenient access for outpatient health care, saving patients’ time and money by providing multiple services in one location. The Legacy at Home plays a critical role in the continuum of care by offering services to seniors in their homes, ensuring a healthy recovery and preventing readmission to the hospital. The Legacy at Home has a skilled medical team of professionals who provide short-term nursing services; physical, occupational, respiratory and speech therapy; wound care; medication and chronic disease management within a patient’s home. In addition, The Legacy at Home has certified nurses’ aides and personal assistants available to help with activities of daily living, respite care and companion services.
